Pre-Check In will save you time when you arrive for your appointment, but it is not required.
If you choose not to complete Pre-Check In, we will have you complete these steps upon arrival for your appointment, so please be sure to arrive at least 15 minutes prior to your appointment time.
To Pre-Check In for your appointment, follow these 3 steps:
1. Log into your portal and review and update your demographic information and medical and ocular history.
2. Review and sign the following 3 documents

HIPAA & Eyeglass Rule
Billing Notice & Optomap
These documents can be completed and sent in a portal message or printed here. To send your documents follow this path: Messages, Inbox, Compose message, Attachments
Or please bring the signed documents with you.
3. Provide an image of your medical insurance card (front and back)
You may upload an image of your card into a secure portal message to our office
